Q. Does Seneca have a consent form for taking videos or photographs of students or posting their assignments online?

There is no official form. Here's one possible form to use for consent to be filmed or photographed. Here is an example of the possible wording of a form seeking permission to post a student's assignment. You can also create your own form including many of the same elements -- name, acknowledgement of the purpose of the photography, levels of permission with which to use the content, what happens once the content is no longer needed, etc.
